Location: Oshkosh Masonic Center Ballroom, located at 204 Washington Ave., Oshkosh, WI 54901

Cost: $25 per ticket (caregivers/family members must also purchase tickets)

Professional Portraits by a local Photographer

Delightful Garden Party refreshments and finger foods

DJ and Dancing

As always, if you require 1:1 assistance to participate, please bring a family member/caregiver to support you

2. Either drop off the completed form at Covey between 9:00 a.m. and 3:00 p.m., Monday- Friday
